License Requirements:
-Graduate of an approved medical college.
-Examination
-One year approved graduate training (US and Canadian schools), two years approved graduate training (foreign schools).

Physician Annual Registration - $192
Physician Application Fee - $200
Exam Administrative Fee -(half day) $50 -(full day) $80
Physician Temporary License - $60
MD Endorsement Fees - $40
MD Certification Fees - $25
MD Late Fees - $60
Residency Permit - $20.00/$15.00
Emeritus Registration - $50
Period:
Annual based on birth month.
Determination:
2 1/2 months.
Other Information:
Special Purpose Examination or current specialty board certification if ten years since licensing exam.
